Activity 8: Assessment Part 2

This is my first time creating a form using Google Forms, but I have taken many surveys that used this tool in the past. It is a very user friendly App. that is easy to navigate. I love that you have the many different options when it comes to creating question types. This will make it more accessible across all content areas. It would also be a great resource to use with parent communication. I could send out surveys as the year progresses to help gather parent input.

Here is the link to a Reading Inventory I created. This will help me learn more about my students; reading habits and home reading atmosphere.

Below is a link to a Unit Studies Assessment I created to review Weathering & Erosion. The questions are branched with support videos for students who answer incorrectly and need review of a concept. I think the grading tool, will be very useful when collecting data for learning targets and PLC work. (Math and Reading topics)

  2. Forms are pretty amazing. I didn't realize how many different things you can do with them, like the branching forms or self-correcting forms. Yours look great - I just noticed a few little typos in the reading inventory. What a fun way to collect information about what they are interested in reading and what their home reading situation is like. Nice work!
